Community Health

The focus of the Tribal Health Outreach program is the prevention of illness, promotion and maintenance of health through the provision of therapeutic services, counseling, education and advocacy services. This is accomplished through assessment and identification of the individual, family and community needs. The establishment of health goals and programs to meet the needs of the community is also a responsibility of this program. Our program is flexible and individualized to meet needs within existing resources and takes into account prevailing economic, cultural, social and geographic characteristics. Some of the service that we provide include:

  • Home Health Visits (with Doctor’s orders)
  • Health Education Outreach
  • Health Fairs
  • Diabetes Classes
  • Smoking Cessation Education
  • Blood Pressure Checks on request
  • Arranging Service needed from Outside Providers
  • Individual Visits Arranged in the Community Health Office
  • Health Outreach to Senior Center
