Appointments
Our Patient Scheduling office works with patients for all of the appointment needs at the FTHC (excluding some Behavioral Health). We also contact our patients to remind them of their upcoming appointments.
How To Make An Appointment
All new patients must register prior to being seen for an appointment. If you are already registered, call for an appointment to our Patient Scheduling hotline at 775-423-3610 or for Behavioral Health call 775-423-9076. If you have not been seen at the FTHC within the past three (3) years, you will be required to re-register (see Eligibility section).
What To Bring To Your Appointment
You will need to bring any and all insurance cards you have if they have changed since they last time you had an appointment. ALL patients must check-in at the FTHC.
At the time of your appointment, please bring a list of medications you are taking (including any supplement or over-the-counter medications you are taking). When you make your appointment, we will inform you of any records or information you need to bring for your appointment.
Children under 18 MUST be accompanied by a parent or legal guardian at every appointment (paperwork for guardians must be on file). A parent or legal guardian is required to accompany children under 18 throughout the appointment.
Arrival For Appointments
All patients must check in to verify/update patient information (address, contact numbers, and insurance) and to allow us to check you into the Electronic Health Records program which notifies your provider that you have arrived.
For your first visit, please plan to arrive 30 minutes prior to your scheduled appointment. This will allow us to prepare your record and collect other required information from you. For subsequent visits, please arrive 15 minutes prior to your scheduled appointment.
Late Arrivals And Missed Appointments
It is the patient’s responsibility to keep track of their appointments. If you are unable to keep an appointment, please call 24 hours in advance.
Patients arriving ten (10) minutes or more after their appointment time are considered late which will result in one of the following (unless the delay is related to an emergency):
- The patient’s appointment will be rescheduled for another date;
- The patient will be scheduled with another provider in an open appointment slot; or
- The patient will be triaged (e.g., by a nursing or dental assistant) to determine the urgency of care; or
- If emergent, the patient will be seen immediately; or
- If urgent, the patient will be placed as a walk-in and worked in with an available provider on a first come first served basis.
Patients who have three (3) or more no-shows in six months’ time are considered excessive and may be put on Limited Access. Limited Access means that you will not be allowed to schedule an appointment for 3 months from the date you are put on Limited Access. Individuals on limited access must call the FTHC on the same day as services are needed in order to be added to the schedule (if there is an available opening in the schedule) and will be triaged and the triage staff will determine if the patient needs to be seen that day.
We understand there may be times when unforeseen circumstances arise, which missing a scheduled appointment(s) or being unable to cancel within the allotted time. In these instances, please contact our Case Manager/Patient Advocate to discuss whether your situation may qualify for a waiver of this provision. Waivers may be approved for situations such as hospitalization, death in the immediate family, or a weather-related emergency.
Walk-In Appointments
The FTHC’s Walk-In Clinic will be open from:
• Mondays …………… 8 – 11 am & 1 – 4 pm
• Tuesdays ………….. 8 – 11 am
• Wednesdays ……. 8 – 11 am
• Thursdays ………… 8 – 11 am
• Fridays ……………… 8 – 11 am & 1 – 4 pm
Patients will be triaged by a member of nursing staff to determine the urgency. Patients are seen based on urgency of their need and may have an extended wait before being seen. Also, our walk-in medical provider will only treat a patient for the item that they related to the Nursing staff (all other treatment and care needs must be done with a scheduled appointment). Patients needing immediate urgent or emergency care may be referred to a local emergency room.
