Eligibility
Our Patient Registration staff have the responsibility to greet incoming patients and handles new patient registration. The following information will help you if you are a new patient or a returning patient who hasn’t been seen here in over three years. We also check for any expired and/or missing documents to make sure the patient’s chart is current and correct.
At each visit, patients will be given a sheet with information showing all of the current data and patients just have to review this and make any corrections (if any) and then sign the bottom of the form and return it to the Patient Registration window.
Who Is Eligible For Services?
American Indians/Alaska Natives (AI/AN) from federally recognized Tribes or descendants from a federally recognized Tribe are eligible for direct services. Direct services include services provided by Fallon Tribal Health Center that are available on-site, unless otherwise noted.
The FTHC does see some non-AI/AN patients for direct care including children of an eligible Indian, a non-Indian woman pregnant with an eligible Indian’s child, a non-Indian in some situations relating to public health hazards and infectious diseases, etc. Please contact the FTHC for more information.
How do I Register?
New patients (or patients that haven’t been seen in over three years) must submit a Patient Registration Packet with copies of all required documentation. A complete Patient Registration Packet must be received prior to scheduling your first appointment. If you don’t have all of the required documentation, we will not be able to see you for services (except if you are experiencing an urgent/emergency issues). If the packet and documents are not provided within 90 days, charts will be inactivated. Patient Registration Packets are available on this website and at the FTHC. Please call us if you would like to receive a Patient Registration Packet by mail. Additional consent forms and authorizations may be required onsite.
Required Documentation
To access direct services available at the FTHC, all patients must provide:
- Birth Certificate
- Social Security Card
- Proof of American Indian/Alaska Native Status:
- Tribal Enrolment/Membership Card
- BIA Certificate of Indian Blood (CIB)
- Proof of Descendancy from an Eligible Indian
- Insurance Card (if applicable)*
- Current Driver’s License/ID Card
- Proof of Residency including the physical address
- Proof of Guardianship or Power of Attorney (if applicable)
- For name changes, (marriage, divorce, etc.), a copy of a marriage certificate or other court document to verify change of name
* All individuals that seek care through the FTHC are required to apply for the governmental benefits for which they qualify; which include but are not limited to:
- VA Benefits
- Medicare
- Medicaid
- Health Exchanges under the Affordable Care Act
- Other Eligible Resources
If the patient’s insurance has expired or has no insurance, you will be referred to our Benefits Office to help facilitate and apply for benefits through the respective agencies. However, insurance coverage is not a requirement to receive direct services.
