CHR Program
Our Community Health Representative (CHR) program works to help individuals and families gain access to comprehensive healthcare services. The goal is to provide American Indians/Alaska Natives (AI/AN) in our service area, with a variety of health services. The CHR is often the first link for the patient to gain access to the health care system. Our CHR Program currently offers the following services:
- Home visits to AI/AN patients who are high–risk elders or those prone to illnesses
- Medication Deliveries from FTHC Pharmacy (please allow 24-48 hours for delivery)
- Patient Transportation Services (refer to “Transportation” under Patient Info tab)
As there is more demand than available CHRs, we have to prioritize our services first to elder’s (50 years & older) and patients who are physically handicapped (as defined by federal/state guidelines). Patient transportation also two additional priority areas including that an appointment is for an approved referrals from the Purchased/Referred Care program and for appointments with specialty physician consultants for diagnostic testing and/or evaluations.
Please note that the CHR Program has a service area that is for the Fallon Paiute-Shoshone Tribe and Churchill County area. The Lovelock and Yomba Tribes have contracted their own CHR program and should be contacted directly by patients in those areas.
